Job summary

Excel Care Wilmington is seeking a Part-Time Social Worker at a Skilled Nursing Facility to support residents and families throughout their stay.

You will assess psychosocial needs and help with discharge planning, adjustment to facility life, and advocacy, coordinating with care teams to ensure emotional and social needs are met.

Qualifications

• Master's degree in Social Work (MSW).
• Active state licensure (LSW/LCSW or equivalent) in good standing.
• Experience in long-term care, skilled nursing, or healthcare setting preferred.
• Strong communication and resident-centered care passion.
• Ability to pass background checks and clearances.
