Job summary

Excelcare at Lewes in Lewes, DE is seeking a compassionate, state-certified social worker to join its Social Services team on a part-time basis (25 hours per week). This role offers meaningful work supporting residents and families through every step of care.

You will complete psychosocial assessments, participate in care planning, coordinate discharge planning, and advocate for residents' rights and quality of life, all while maintaining accurate documentation in compliance with regulations.
