Job summary

Complete Care is seeking a part-time Social Worker (LSW / CSW / LCSW) to join our skilled nursing and rehab team in New Jersey. You will coordinate admissions, discharges, and transition planning while providing emotional support to residents and families during critical care transitions.

The role requires an active NJ license and 1–2 years in healthcare, with a focus on discharge planning, family advocacy, and interdisciplinary collaboration within a supportive leadership team and

Qualifications

• Current New Jersey LSW, CSW, or LCSW license required.
• Minimum 1–2 years of healthcare setting experience, preferably skilled nursing, long-term care, or rehab.
• Experience with discharge planning, Medicaid/Medicare, and MLTS preferred.
• Understanding of CMS regulations and Department of Health survey process preferred.
• Strong communication, documentation, and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Participate in interdisciplinary care plan meetings.
• Educate residents and families regarding Advanced Directives.
