Job summary

CalOptima Health seeks a Supervisor Social Work (PACE) to lead the PACE Center social work team, oversee clinical direction, and manage care coordination for elderly participants. This role emphasizes supervisory duties, collaboration with the IDT, and ensuring high-quality social work services in a community-based setting.

Responsibilities include overseeing authorization services, coordinating home care, and participating in care planning, with travel within Orange County as needed.

Qualifications

• Master's degree in social work PLUS 3 years of social work experience in a health-related field.
• 1 year of experience working with the frail or elderly population within the last 3 years.
• 1 year of progressive leadership experience.
• Access to transportation for work away from the primary office ~15% of the time required.
• CPR and First-Aid Certification required or to be obtained within 6 months.

Responsibilities

• Oversee supervisory functions and supervision of the Social Work team.
• Provide clinical direction to the social work team and associates seeking licensure.
• Lead weekly meetings to discuss high risk cases and provide guidance.
• Oversee documentation for compliance with industry and departmental standards.
• Coordinate home care services, assessments, and authorizations.
• Participate in IDT assessments, care planning, and periodic reassessments.
• Develop relationships with community agencies and facilities for referrals.
• Ensure services are implemented per plan of care and quality measures.
