Qualifications

• Master's Degree in Social Work required.

Responsibilities

• Oversee direct social work and case management services for San Diego PACE participants.
• Supervise and train social workers and case managers; ensure adherence to policies and licensing requirements.
• Coordinate psychosocial assessments, care planning and re-assessments (min. every 6 months).
• Participate in hiring, supervise staff, and manage caseload allocations.
• Ensure documentation is accurate, timely and compliant with regulations.
• Provide clinical and crisis management support to participants, families, and staff.
