Job summary

Neighborhood Healthcare PACE in Murrieta, CA is seeking a Social Work Manager to lead social work services within the PACE program. This onsite role partners with clinical teams and leadership to ensure high-quality, person-centered care and compliant operations.

You will supervise staff, coordinate IDT processes, oversee assessments and care plans, and drive quality improvement while building community partnerships to support participant outcomes.

Qualifications

• BLS certification required upon hire and maintained.
• CPR and First Aid certification required for all PACE employees.
• Three years social work experience with elderly/frail adults; four preferred.
• Two years supervisory experience; elder population preferred.
• Valid CA Driver’s License and auto insurance.
• California ACSW preferred or ability to obtain.

Responsibilities

• Provides leadership, supervision, and support to Social Work staff.
• Oversees psychosocial assessments, reassessments, and care plan coordination.
• Ensures compliance with CMS/DHCS and regulatory requirements.
• Drives process improvements to enhance outcomes and workflows.
• Facilitates IDT meetings and coordinates interdepartmental collaboration.
• Builds relationships with community partners and referral sources.
