Job summary

AltaMed Health Services Corporation in Lynwood is seeking a Social Work Supervisor for the PACE program. You will oversee the social work staff, coordinate assessments and care plans, and support interdisciplinary collaboration to provide person-centered care.

This role requires an MSW, 2+ years of supervisory experience in health or human services, and strong bilingual abilities. You will also guide on behavioral health, life planning, and end-of-life care, while ensuring compliance with state

Qualifications

• Master's degree in social work (MSW) required; LCSW preferred.
• Minimum of 2 years' supervisory experience in health-related or human services field.
• Minimum of 2 years in case management/social advocacy with geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Oversee and supervise PACE site social work team and direct reports.
• Coordinate assessments and care plans; support family conferences and referrals.
• Ensure documentation, onboarding, orientation, and performance evaluations.
• Provide guidance on behavioral health, life planning, and end-of-life care.
