Job summary

CalOptima Health seeks an Activity Coordinator for its PACE program in California. You will develop and coordinate individual and group recreational activities for participants, monitor progress, and align activities with care plans and regulatory requirements.

You will collaborate with the Interdisciplinary Team, clinicians, and community partners to deliver engaging programs that promote dignity and quality of life for seniors in our care.

Qualifications

• Bachelor's degree in recreational therapy or a related field; 2 years of elderly care experience.
• CPR and First-Aid Certification required or must be obtained within six months.
• Must obtain Activity Coordinator State certification within 90 days of hire.
• Bilingual in English and at least one threshold language (Arabic, Farsi, Chinese, Korean, Russian, Spanish, Vietnamese) required.

Responsibilities

• Coordinate and oversee the activity program and calendar.
• Develop individual and group recreational activities for participants.
• Collaborate with IDT and other staff to meet care plan goals.
• Monitor and revise activities to align with regulations and goals.
• Coordinate community outings and events when possible.
