Home Base, Inc. in Huntington, WV is seeking a Paraprofessional Assistant Coordinator to support social services for at-risk children and families.

You will help build service plans, conduct home visits, and coordinate with DHHR case managers while adhering to ethics and guidelines. The position starts part-time with potential to become full-time, offering health, dental, vision, and other benefits after transitioning to full-time.

Qualificações

• High School Diploma or GED required.
• Must pass Fingerprint Background Check (CIB) and APS/CPS.
• Ability to work evenings and weekends.
• Valid Driver's License and reliable transportation.
• Experience with Microsoft Office is essential.
• Strong communication and documentation skills.

Responsabilidades

• Establish personalized service plans for clients and target deficits and strengths.
• Open cases within mandated timeframes and schedule visits at least weekly.
• Provide services within the HIPAA/ ethics guidelines and maintain records.
• Communicate with DHHR case managers to relay important information.
• Maintain organized client records with intervention notes and deadlines.
