Home Base, Inc. is seeking Paraprofessional Assistant Coordinators to deliver in-home social services to children and families. You will develop personalized plans, conduct weekly visits, and coordinate with DHHR case managers to support family stability.

Qualifikationen

• High School Diploma or GED required.
• Must be able to pass Fingerprint Background Check (CIB) and APS/CPS screening.
• Ability to communicate effectively with families, children, case workers, and supervisors.
• Ability to work evenings and weekends.
• Must have reliable transportation and a valid driver’s license and insurance.
• Proficiency with Microsoft Office and general computer skills.
• Organizational skills are essential and time management is a must.

Aufgaben

• Establish and manage personalized service plans for clients.
• Schedule weekly visits with clients.
• Provide services within defined guidelines and ethics.
• Maintain accurate client records and intervention notes.
• Communicate with DHHR case managers and supervisors.
• Attend staff meetings and trainings as required.
• Engage in multidisciplinary team meetings and advocate for clients.
• Testify in court regarding client progress and case outcomes.
