Home Base, Inc. in West Virginia seeks Paraprofessional Assistant Coordinators to provide social services, observational and instructional support to children and families.

The role focuses on safety, well-being, and in-home services to improve parental skills and family stability, including foster care support. Formación

• Must have High School Diploma or GED.
• Able to pass Fingerprint Background Check (CIB) and APS/CPS.
• Effective communication with families, children, case workers, and supervisors.
• Ability to work evenings and weekends.
• Organizational and documentation skills are essential.
• Valid driver's license and reliable transportation.

Responsabilidades

• Establish personalized service plans for clients and interventions targeting deficits.
• Schedule visits with clients at a minimum of once weekly or as required.
• Maintain documentation within intervention notes and deadlines.
• Communicate with DHHR case managers regarding client updates.
• Participate in Multi-Disciplinary Team meetings and court testimony.
• Uphold NASW and ASO codes of ethics in professional matters.
