Job summary

Home Base, Inc. in Summersville, West Virginia seeks a dedicated Paraprofessional Assistant Coordinator to deliver in-home social services, support vulnerable children and families, and help preserve family bonds during court-ordered periods.

You will establish personalized service plans, schedule weekly visits, document interventions, and collaborate closely with DHHR case managers. This role starts part-time with the potential to become full-time and offers a competitive benefits package.

Qualifications

• High School Diploma or GED required.
• Pass fingerprint background check and APS/CPS screening.
• Ability to communicate effectively with families, children, and staff.
• Able to work evenings and weekends.
• Proficient in Microsoft Office and computer skills.

Responsibilities

• Establish personalized service plans and interventions.
• Schedule weekly visits with clients.
• Maintain accurate client records and documentation.
• Participate in multidisciplinary team meetings and court testimony.
• Support services such as Respite and independent living/skills building.
