Job summary

Home Base, Inc. seeks a Paraprofessional Assistant Coordinator to provide social services for children and families. This role involves in-home support, foster care collaboration, and case management under supervision, with emphasis on trauma-informed approaches and ethical practice.

You will establish service plans, schedule visits, and maintain client records while working evenings and weekends as needed in West Virginia. A high school diploma and valid driver’s license are required.

Qualifications

• High School Diploma or GED is required.
• Must pass a Fingerprint Background Check (CIB) and an APS/CPS screen with no negative information.
• Ability to communicate effectively with families, children, case workers, and supervisors.
• Ability to work evenings and weekends.
• Organizational and documentation skills are essential.
• Time management skills are required.
• Must have a valid Driver’s License and Insurance and reliable transportation.
• Experience with Microsoft Office and basic computer skills.

Responsibilities

• Establishing personalized service plans for clients and providing systematic interventions targeting deficits and capitalizing on strengths.
• Opening cases within mandated timeframes; scheduling visits with clients at least weekly or as specified.
• Providing services within applicable guidelines and adhering to professional ethics.
• Documenting all contacts with clients, including intervention tactics and responses.
• Communicating with DHHR case managers to provide updates and relevant information.
• Maintaining orderly client records with intervention notes and attending team meetings.
• Participating in monthly clinical supervision and general staff trainings.
• Offering supportive services such as Respite and Independent Living/Skills Building where appropriate.
