You will establish personalized service plans, schedule visits, and coordinate with DHHR case managers, adhering to NASW and ASO standards. Qualifications include a high school diploma, a clean background, and reliable transportation.

Qualifications

• Must pass a Fingerprint Background Check (CIB) and APS/CPS.
• High School Diploma or GED is required.
• Able to communicate effectively with families, children, case workers, and supervisors.
• Ability to work evenings and weekends.
• Strong organizational and documentation skills.
• Time management and professionalism are essential.
• Must have a valid driver's license and insurance.
• Reliable transportation is required.
• Experience with Microsoft Office and computer skills are essential.

Responsibilities

• Establish personalized service plans for clients and provide targeted interventions.
• Open cases within mandated timeframes and schedule weekly visits.
• Provide services per applicable guidelines and codes of ethics.
• Maintain client records with intervention notes and deadlines.
• Communicate with DHHR case managers for updates and client information.
• Participate in multidisciplinary team meetings and court testimony when needed.
• Promote respectful communication and professional practice, seeking ongoing improvement.
