Qualifications

• Must have a High School Diploma or GED.
• FingerPrint background (CIB) and APS/CPS screen required.
• Valid Driver’s License and Insurance.
• Must have reliable transportation.
• Experience with Microsoft Office is essential.

Responsibilities

• Establish personalized service plans for clients and provide targeted interventions.
• Schedule visits with clients at least weekly as directed by the DHHR case manager.
• Document all contacts and intervention tactics with clients.
• Maintain organized client records with notes and timelines.
• Collaborate with DHHR case managers and participate in multidisciplinary meetings.
• Testify in court regarding client progress and case closure, as needed.
