Job summary

PACS in the United States seeks a Social Services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility's social service programs. You will ensure the medical and psychosocial needs of residents are met, working with administration, medical and nursing staff to plan services.

You will help develop policies, maintain progress notes, coordinate with external agencies, and advise on Medicare/Medicaid information and financial assistance

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent. Associate or Bachelor’s Degree preferred but not required.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs.
• Meet with administration, medical and nursing staff, and other related departments in planning social services.
• Assist in developing and coordinating department policies and procedures.
• Review department policies and procedures at least annually and participate in changes.
• Provide information to residents/families about Medicare/Medicaid and financial assistance programs.
• Record and maintain regular Social Service progress notes indicating response to treatment plans.
• Coordinate social service activities with other departments as necessary.
• Assist residents with achieving the highest practicable level of self-care, independence and well-being.
• Discharge planning with appropriate agencies and entities.
