Job summary

PACS is seeking a dedicated Social Services Assistant to support planning, development, and evaluation of resident social programs, ensuring compliance with federal, state, and local standards.

You will collaborate with administration, medical staff, and families, maintain progress notes, assist with discharge planning, and help residents achieve maximum independence and psychosocial well-being.

Qualifications

• Must have a high school diploma or equivalent
• Associate or Bachelor’s Degree preferred but not required
• Healthcare experience preferred
• Ability to read, analyze, and interpret documents
• Ability to write reports and communicate effectively

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs
• Meet with administration, medical and nursing staff to plan social services
• Develop and coordinate department policies and procedures
• Maintain regular social service progress notes
• Coordinate discharge planning and community resource referrals
• Assist residents in achieving maximal self-care, independence, and well-being
