Job summary

PACS seeks a Social Service professional to plan, develop, organize, implement, evaluate, and direct our facility's social service programs in alignment with federal, state, and local standards. You will ensure the medically related emotional and social needs of residents are met on an individual basis.

Responsibilities include collaborating with administration, medical staff, and other departments; developing policies, coordinating services, and maintaining progress notes while safeguarding

Qualifications

• High school diploma or equivalent; associate/bachelor's degree preferred but not required.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business material.
• Ability to write reports and procedures; present information to managers/ staff.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical, and nursing staff to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Maintain regular social service progress notes and report to the Director.
• Coordinate social service activities with other departments.
• Assist with discharge planning and community resources.
