Job summary

PACS is seeking an experienced Social Services support professional to assist in planning and directing our facility's social service programs, ensuring residents' medically related social and emotional needs are met in accordance with standards.

The role involves collaboration with administration, medical staff, and community agencies to identify and address residents' needs, coordinate discharge planning, and maintain thorough progress notes and records.

Qualifications

• The candidate must possess a high school diploma or equivalent; an Associate or Bachelor’s degree is preferred but not required.
• Prior healthcare experience is preferred.
• Ability to read, analyze, and interpret general business materials and regulations; ability to present information.

Responsibilities

• Assist in planning, developing, organizing, implementing and directing social service programs.
• Coordinate with administration and medical staff to plan social services.
• Develop and coordinate department policies and procedures.
• Review and improve policies annually and implement changes as needed.
• Provide information to residents on Medicare/Medicaid and other financial assistance programs.
