PACS seeks a Social Services professional to assist in planning, development, and direction of the facility’s social service programs, ensuring compliance with federal, state, and local standards and policies.

The role supports residents’ medically related social needs, collaborates with administration and clinical staff, and contributes to policy development and program evaluation within the facility.

Qualificações

• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Responsabilidades

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs of the facility.
• Meet with administration, medical and nursing staff to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Review department policies, at least annually, and participate in changes.
• Advise on identifying medically related social and emotional needs of residents.
• Coordinate social service activities with other departments as necessary.
• Assist residents in achieving the highest practicable level of self-care and well-being.
• Provide information to residents/families about Medicare/Medicaid and other financial assistance programs.

Formação académica

High school diploma or equivalent

Associate or Bachelor’s Degree preferred but not required
