Job summary

PACS is seeking a Social Services professional to assist in planning, developing, and directing the social service programs for a post-acute facility. You will collaborate with administration, medical staff, and nursing teams to implement compassionate, compliant care plans for residents.

You will assist with policy development, discharge planning, patient resource referrals, and staff consultation to address social, emotional, and financial needs. Strong communication is essential.

Qualifications

• High school diploma or equivalent; Associate/Bachelor’s degree preferred but not required.
• Prior health care experience preferred.
• Ability to read, analyze, and interpret general business materials; ability to write reports.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services and develop policies.
• Review and update department policies and procedures; annual reviews and changes.
