Job summary

The Salvation Army in Trenton, NJ seeks a Social Services Case Manager to provide case management for the Trenton Citadel social service programs under supervision of the Corps Officer and Divisional Headquarters Human Services staff.

You will perform intakes and assessments, develop action plans, coordinate services with partner agencies, enroll eligible families in Pathway of Hope, maintain confidential records, and assist with reporting and volunteer oversight to support community needs.

Qualifications

• Associate’s degree in social work or related field from an accredited program.
• Bachelor’s degree in social work preferred.
• Minimum 1–2 years of case management experience.

Responsibilities

• Perform intakes, interviews, and assessments of clients’ needs and follow up case management with households.
• Identify and interact with case management clients; assess needs and develop action plans using a strengths-based approach.
• Coordinate services with partner agencies for a holistic service approach.
• Support Pathway of Hope by enrolling eligible families with children under 18.
• Ensure timely submission of records and reports in databases for programs and funding sources.
• Maintain accurate, confidential case records and organization of paperwork.
• Assist with lobby upkeep, switchboard, and voicemail to ensure timely assistance.
• Assist Officers in overseeing volunteers and community service workers.
• Prepare and maintain Salvation Army statistical reports for timely submission.
