Job summary

Intensive Treatment Systems, LLC is seeking a SMI Services Case Manager to provide comprehensive case management to adults with serious mental illness. You will collaborate with a multidisciplinary team to plan services, coordinate care, and advocate for client access to resources.

Responsibilities include assessments, treatment planning, crisis intervention, and ensuring quality documentation. Field and office work require reliable transportation and a compassionate, client‑centered approach.

Qualifications

• 1–2 years of experience working with individuals with serious mental illness or in related social services.
• CPR and Crisis Response training certificate.
• Valid driver’s license and reliable transportation.
• Ability to pass background checks and required screenings.
• Strong interpersonal, organizational, and communication skills.

Responsibilities

• Conduct assessments and create treatment plans identifying client needs, strengths, goals, and barriers.
• Develop individualized service plans with clients, family members, and treatment team.
• Coordinate and monitor service delivery including referrals to mental health, medical, housing, employment, and social services.
• Maintain regular contact with clients via home visits, calls, and meetings.
• Provide psychoeducation, supportive counseling, and crisis intervention as needed.
• Advocate for clients to access entitlements, benefits, and appropriate services.
• Collaborate with psychiatrists, therapists, housing providers, law enforcement, and others to support care.
• Maintain accurate documentation in line with agency policies and regulations.
• Participate in team meetings, trainings, and case consultations.
