Zusammenfassung

The Cathedral Center is seeking a Shelter Case Manager to support women and families during emergency shelter stays in Milwaukee, focusing on housing, income, health and wellbeing goals. You will perform assessments, develop case plans, and coordinate with community resources to help clients move into stable housing.

Responsibilities include crisis intervention, data entry, reporting, and collaboration with partner agencies.

Qualifikationen

• Bachelor’s degree in counseling, social work or related areas with emphasis on family systems theory, trauma informed care or families in poverty
• Experience serving individuals suffering from addictions, mental illness and/or poverty and homelessness
• Effective skill in use of Excel, Word and other Microsoft applications, as well as comfort in learning an internet-based case management program needed
• Fluency in written and spoken English

Aufgaben

• Provide comprehensive assessments, case planning and follow up services to women and families experiencing housing crises and homelessness
• Crisis intervention and problem solving among client’s personal supports and networks
• Facilitate individual, family and group short term supportive sessions to assist clients in achievement of goals for independence and utilizing community resources effectively
• Maintain accurate, confidential, professional and up to date documentation of all client services
• Attend and participate in weekly staff meetings and client reviews
• Collaborate with partner agencies and participate in community meetings relative to the homeless provider network
