Job summary

Catholic Community Services via Bertha's Place Shelter seeks a Case Manager to assist guests in exiting to permanent housing. You will assess housing needs, build stability plans, and provide supportive case management to residents in a 24/7 shelter setting.

Strong communication and organizational skills are essential. The role emphasizes trauma-informed care, collaboration with the shelter team, and compassionate support to individuals experiencing homelessness.

Qualifications

• AA in a human or social services field, or 2 years of shelter generalist experience.
• Excellent interpersonal, oral and written communication skills.
• Excellent organizational and time management skills.
• Strong computer skills including Microsoft Word and database experience.
• Ability to work with a diverse spectrum of high-need individuals.
• Must have reliable transportation, valid driver’s license and automobile insurance.
• Demonstrates the attitudes, knowledge and skills to deliver culturally competent services.
• Background clearances required prior to offer of employment.
• Ability to work independently and as part of a team.

Responsibilities

• Assess housing needs using Motivational Interviewing and a Progressive Engagement Approach; build trust and identify barriers.
• Develop and initiate housing stability plans with steps to secure housing.
• Provide case management to Bertha's Place clients; review progress and support action steps.
• Keep accurate case management notes and prepare weekly status reports.
• Attend weekly team meetings and other meetings as required by supervisor.
• Perform other duties as assigned.
