Job summary

Catholic Charities of the Archdiocese of Washington is seeking a Shelter Case Manager to provide onsite case management to help clients exit shelter, document progress within 48 hours, and connect clients to benefits, housing resources, and community referrals. You will work with a multidisciplinary team to address barriers to housing and employment and support positive shelter exits.

• Ability to analyze situations accurately and make informed decisions.

Responsibilities

• Interview clients for direct service needs and develop case plans.
• Reevaluate goals with clients periodically and adjust plans.
• Lead individual and group meetings as needed and coordinate with multidisciplinary teams.
• Make referrals for emotional, substance abuse, legal, medical, and housing needs.
• Create and maintain case records and progress reports.
• Ensure a hospitable, customer-oriented client environment and complete required documentation.
