Job summary

The Salvation Army Sherman Corps is seeking a dedicated Case Manager to oversee an assigned caseload within our emergency day shelter program. You will establish program plans and monitor client progress through regular meetings with clients and staff, advocating to access essential services to help clients cope with their environment.

Responsibilities include developing tailored case plans, mentoring and counseling clients, referring to Salvation Army programs, and maintaining accurate case

Qualifications

• Associate's degree or two years of college coursework in Social Work, Behavioral Science, or a related field.
• Two years experience working in a social or public service environment with case management.
• Valid State Driver's License.

Responsibilities

• Interviews, accepts, and provides comprehensive, long-term, structured, case management services for an assigned caseload of clients participating in an established life management program.
• Creates a case plan based on the client history to determine effective program plans for success.
• Develops goals and evaluates client progress through mentoring and counseling sessions with client and/or staff.
• Serves as advocate for clients to obtain services and ensures compliance with funding requirements.
• Refers clients to Salvation Army programs or other agencies based on client needs.
• Assists in onsite food pantry and maintains case records for all assigned clients.

Education

Associate degree or two years of college coursework in Social Work or related field
