Job summary

PLAN is seeking a licensed social worker (LSW/LCSW/LICSW) to join our team as a Service Coordinator. You will work with individuals with disabilities to ensure trust funds improve quality of life while protecting public benefits eligibility.

You will maintain ongoing contact with participants, assess spending plans, and collaborate with the Trust Operations team. Travel within MA/RI and strong knowledge of Medicaid and housing benefits are essential.

Qualifications

• Valid Social Work License in Massachusetts (LSW, LCSW, or LICSW) (dual license MA/RI, a plus).
• Experience working with people 65 and older, people with physical disabilities, and people with mental health and/or intellectual challenges.
• Current Driver’s License.
• Must have own car and be willing to travel.
• Familiarity with resources and support services.
• Excellent interpersonal skills.
• Ability to problem-solve and advocate.
• Ability to communicate effectively with a broad range of individuals and organizations.
• Excellent organizational skills.
• Ability to work independently and interdependently.

Responsibilities

• Initial meeting of new trust participants to explain the trust and use of trust funds; assess participants’ living arrangements, strengths, and weaknesses; and develop a spending plan and/or life plan
• Ongoing contact with trust participants and assessment of spending plans
• Work with Trust Operations team to review and approve beneficiary requests
• Resource development and identification
• Maintain current knowledge of public benefits, such as Medicaid, disability benefits and housing.
• Participation in PLAN events for beneficiaries and families
• Aid with program development and innovation in MA and RI
