Job summary

Care Lync in South Carolina is seeking a dedicated Support Coordinator to manage individualized care plans, coordinate services, and monitor outcomes for adults with disabilities enrolled in Medicaid Waiver programs.

You will develop Person-Centered Service Plans, connect clients with housing, medical, and community resources, and ensure compliant, confidential care while traveling to participant homes as needed.

Qualifications

• BS/BA degree in related field.
• Minimum 3 years related experience providing case management for individuals with Intellectual Disabilities, Autism Spectrum Disorders, or Physical Disabilities.
• CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

• Complete assessments to identify each participant needs and desires for service delivery.
• Develops a Person-Centered Service Plan for each participant and modify annually or with significant changes.
• Ensure the participant choice of providers by providing information for fully informed decisions.
• Ensure the Behavioral Support Plan and Crisis Plan are consistent with the ISP.
• Report all incidents of participant abuse, neglect or exploitation immediately to supervisor/director.
• Actively coordinate with other care coordinators and entities to ensure seamless service coordination.
• Research and develop alternative solutions to participants needs.
• Maintain confidentiality of sensitive personal health information.
• Attend required trainings and recertification classes.
• Confer with supervisor to understand participant needs and limitations.
• Travel to participants as needed (about 50%).

Education

BS/BA degree in related course work

Minimum 3 years related experience

CPR/First Aid certification

Valid Driver's License
