Job summary

United Counseling Service in Vermont seeks a Service Coordinator to develop, implement, and monitor services for individuals with intellectual or developmental disabilities. Under supervision, you will manage ISA planning, coordinate with a multi-disciplinary team, and advocate for clients to ensure high-quality supports in the community.

The role requires a HS diploma (BA preferred) and strong interpersonal skills; travel to client homes is expected.

Qualifications

• High School Diploma or equivalent; service coordination experience preferred.
• At least one year of developmental services experience or a BA in human services.
• Strong interpersonal, communication, and writing skills; proficient with Windows, typing, email navigation.

Responsibilities

• Facilitate person-centered planning and ISA management.
• Document and collect data; assist clients in identifying strengths and goals.
• Coordinate services with physicians, therapists, and other providers.
