Requires a Bachelor's degree (or higher) and experience with Intellectual Developmental Disability population; Master's preferred with 0 years. Strong communication, organization, and computer skills are essential for success in this role.

Qualifications

• Required: Bachelor's or advanced degree from an accredited college/university.
• Preferred: Degree in social, behavioral, or human services fields.

Responsibilities

• Meets with developmental disability customers at their homes to assess needs and determine required services.
• Explains services to customers based on needs and makes recommendations.
• Enters information into the computer system and tracks service delivery.
• Helps customers access services to improve quality of life.
• Meets with customers monthly or quarterly to evaluate services and identify new needs or interventions.
• Liaises with service providers to arrange and advocate for services, ensuring customer needs are met.
• Assists during crises and links customers to prevent future crises.
• Acts as primary customer contact to ensure satisfaction.
• Performs other duties as assigned.
