Job summary

Hill Country Mental Health and Developmental Disabilities Centers is seeking a Service Coordinator in Hays County to assess needs, advocate for clients, and develop Person Directed Plans. You will train skills, manage crises, and maintain compliant documentation while coordinating with providers to support person-centered outcomes.

The role requires degrees or relevant experience, a valid Texas driver’s license, CPR and ABM/BP training within 3 months, and strong communication abilities to

Qualifications

• Bachelor's or advanced degree from an accredited college or university.
• Two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
• CPR certification and Prevention and Management of Aggressive Behavior training within 3 months.

Responsibilities

• Assess individuals' needs and collaborate to develop and execute life management and Person Directed Plans.
• Provide relevant skills training to improve daily life.
• Collaborate with service providers to liaise for effective care and positive outcomes.
• Maintain compliant documentation with correct grammar.
• Schedule and conduct meetings following state and company guidelines.
• Respond to crisis intervention with minimal supervision.
• Maintain confidentiality per HIPAA regulations.
