Central-Counties-Center-For-Mental seeks a Service Coordinator to manage consumer plans under HHSC waivers. You will develop PDPs, lead planning meetings, and coordinate with internal and external providers to ensure service delivery and documentation are timely and accurate.

The role requires a Bachelor’s or an associate degree in related fields, and the ability to drive within the five-county service area. Remote work approval after six months may be possible.

Formación

• Trauma-informed care knowledge and cultural sensitivity.
• Ability to facilitate planning team meetings and PDP development.
• Proficient in documenting in EHR and MS Word/Excel.
• Strong communication and time management abilities.

Responsabilidades

• Assess consumer needs and develop the Person Directed Plan (PDP).
• Coordinate planning meetings and monitor plan progress.
• Complete timely documentation and progress notes in the EHR.
• Maintain professional relationships with clients, families, and providers.
