Metrocare Services in Dallas seeks a Wraparound Care Coordinator to provide psychosocial/rehabilitative services for individuals with serious mental illness, developmental disabilities, and related needs. You will work within an interdisciplinary team to develop recovery plans, coordinate care, and support successful community reintegration.

The role involves on-call coverage, crisis intervention, transportation coordination, and collaboration with families and providers to maximize functioning

Qualifications

• Bachelor’s degree in human services with 1–3 years’ MH/MR experience.
• Preferred: Master’s degree in human services.
• Driver license required.

Responsabilités

• Provide overall service coordination, psychosocial rehabilitation, emergency services/assessment, referral, transportation, linkage, and advocacy to consumers with varying needs.
• Formulate individual recovery plan based on assessment findings on all new admissions.
• Develop measurable objectives and goals agreed with the consumer.
• Document all attempts to involve consumers, service providers, and caregivers in the service plan process.
• Review service plans every six months or as clinically indicated.
• Participate in daily interdisciplinary team staffing and present psychosocial assessment findings.
