Job summary

Alleghenies United Cerebral Palsy (AUCP) is hiring a Service Coordinator to support individuals and their teams in developing and updating ISP plans, coordinating services, and ensuring compliance with AUCP standards. This role emphasizes client-centered service delivery, strong communication, and teamwork.

Hybrid/remote work with PA visits is required. Requirements include a Bachelor's in Social Work or related field and knowledge of Medicaid LTSS/HCBS.

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS strongly preferred.

Responsibilities

• Collaborate with clients and their teams to develop ISP based on goals and priorities.
• Regularly update service plans to reflect life changes and needs.
• Coordinate and monitor services with paid providers and community resources to ensure plans are followed.
• Maintain open communication with support teams to address concerns promptly.
• Ensure compliance with program regulations, documentation, timelines, and billing requirements.
• Travel within the region to meet with participants for ISP delivery.
