Job summary

Alleghenies United Cerebral Palsy (AUCP) seeks a Service Coordinator to develop, update, and manage individualized service plans for adults with disabilities. You will collaborate with clients and teams to ensure supports are delivered by qualified staff and community resources in a compliant, person-centered manner.

The role is hybrid/remote with visits in PA; candidates must reside in PA or be willing to travel within PA.

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS preferred.
• Experience working with individuals with disabilities or special needs.

Responsibilities

• Develop individualized service plans (ISP) with clients.
• Regularly update service plans to reflect life changes and needs.
• Coordinate services with paid providers and community resources to meet individuals' needs.
• Maintain open communication with support teams to address concerns and updates.
• Ensure all work complies with program regulations and AUCP standards, including documentation, timelines, and billing.
