Job summary

Alleghenies United Cerebral Palsy (AUCP) seeks a Service Coordinator to develop, update, and manage individualized service plans with clients and their support teams, ensuring access to paid providers or community resources while maintaining compliance and clear documentation. This hybrid/remote role requires PA travel for home visits and collaboration with an interdisciplinary team.

A Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field is preferred, with

Qualifications

• Bachelor’s degree in Social Work, Psychology, Education, Human Services, or a related field.
• Knowledge of Medicaid Waiver programs, LTSS, and HCBS is strongly preferred.
• Excellent organizational, communication, and documentation skills.

Responsibilities

• Develop individualized service plans (ISP) with clients and support teams.
• Regularly update service plans based on life changes and needs.
• Coordinate services with paid providers and community resources.
• Ensure plans are implemented by staff and compliant with AUCP standards.
• Maintain open communication with support teams and meet billing requirements.
• Travel within the region to meet participants and complete home visits.
