Alleghenies United Cerebral Palsy (AUCP) is hiring a Service Coordinator to work with individuals and their support teams to develop, update, and manage personalized service plans. This hybrid/remote role requires travel for home visits in PA and a commitment to client-centered care and compliance.

You will coordinate with paid providers and community resources to ensure services meet needs, are authorized, and implemented by qualified staff, while maintaining strong communication with the team.

Qualifications

• Bachelor's degree in Social Work, Psychology, Education, Human Services, or related field.

Responsibilities

• Collaborate with clients and teams to develop individualized service plans (ISP).
• Regularly update service plans to reflect changes and needs.
• Coordinate and monitor services with providers to ensure plans are followed and resources are used appropriately.
• Maintain open communication with support teams to address concerns and updates.
• Ensure compliance with regulations, documentation, timelines, and billing requirements.
