Job summary

Kintegra Health is seeking an Activity Coordinator in Shelby, NC to plan, coordinate, and implement therapeutic activities for Senior TLC participants in a home and community-based care model. You will work under the supervision of the Adult Day Health Coordinator or Center Manager.

Education must include a High School Diploma or GED and current certification/licensure in activities; experience with frail elderly populations is required. National/NC Activity Director Certification is preferred.

Qualifications

• Education: High School Graduate or GED minimum and current certification/licensure in activities.
• Experience: 1 year in social or recreational program for frail elderly population within past 5 years.
• Licensure: National and/or NC Activity Director Certification.

Responsibilities

• Participates as a member of the Interdisciplinary Team (IDT)/Care Team and communicates participant changes.
• Records, signs, and dates documentation in the participant's medical record per policy.
• Helps to develop, implement, and supervise the Activity Program in the center.
• Plans and provides both group and individual activities.
• Develops specific programs as part of the plan of care to include therapeutic and/or social activities.
• Serves as a resource to staff and coordinates volunteer activities at the center.
• Builds a sense of community and a warm social atmosphere for participants.
• Plans a monthly/daily calendar for the ADHC/Social Center with CNAs.
