Kintegra Health is seeking an Activity Coordinator for the Senior TLC program to plan, coordinate, and implement therapeutic activities in a home and community-based model of care. The role reports to the Adult Day Health Coordinator or Center Manager.

Responsibilities include leading the activity program, conducting assessments, documenting care, and coordinating with the IDT. Requires a High School diploma/GED and an Activity Director Certification, plus at least 1 year in elder care services.

Formación

• High School Diploma or GED and current certification/licensure in activities.
• Minimum 1 year of experience in social or recreational program for a frail elderly population within the past 5 years.
• National and/or NC Activity Director Certification required.
• Must be able to work effectively in a team environment and understand aging psychosocial issues.

Responsabilidades

• Participate as a member of the Interdisciplinary Team (IDT)/Care Team and conduct assessments and updates.
• Document in the participant's medical record in a timely manner as per policy.
• Develop, implement, and supervise the Activity Program at the center.
• Plan and provide both group and individual activities.
• Coordinate with staff and community resources for activities and volunteers.
• Foster a warm social atmosphere and support system for participants.
