Job summary

Kintegra Health seeks an On-Site Specialist Liaison in Shelby, NC to coordinate upcoming on-site specialist visits and handle post-visit follow-up with referring providers. The role reports to the Health Information Management (HIM) Manager and requires liaison with multiple teams to ensure timely, compliant documentation.

Minimum education is a high school diploma or GED; 1+ year working with frail elderly clients is preferred. Certifications such as MA/CMA/RMA/CCMA/CNA/LPN are required.

Qualifications

• High school diploma or GED required.
• 1+ year experience with frail elderly population; OJT acceptable.
• Certification/registration as Medical Office Assistant, RMA, CMA, CCMA, CNA or LPN required.

Responsibilities

• Monitors Service Tracking for on-site referrals and develops schedules for each on-site provider visit.
• Prepares documentation and sends it to the on-site provider in the expected timeframe (see On-Site Specialist Documentation and Timeframes).
• Prepares space in the day center/clinic prior to the visit.
• Communicates with participant/caregiver about upcoming appointments at defined intervals.
• Uploads signed consent forms to the participant's chart and sends to the on-site provider.
