Job summary

Kintegra Health in Shelby, NC, is seeking a Social Worker (MSW) to provide direct social work case management services to Senior TLC participants in a home and community-based model of care. You will screen participants, coordinate referrals, and make home visits, with frequent travel and reporting to the Center Manager.

Requires Master's Degree in Social Work, 2 years in health-related field preferred, and 1 year with frail elderly; must have a valid NC driver's license and vehicle,

Qualifications

• Master's Degree from an accredited School of Social Work is required.
• 2 years experience in a health-related area preferred.
• Minimum 1 year experience working with frail elderly population.
• Valid North Carolina Driver's License (NCDL) and vehicle.
• Immunizations up to date before engaging in direct participant contact.

Responsibilities

• Provide direct social work case management services for Senior TLC participants.
• Screen participants, coordinate referrals, and link to community resources.
• Conduct home visits, which may require frequent travel, and report to Center Manager.
• Maintain written records and participate as a member of the interdisciplinary team.
