Job summary

Qualifications

• High School Diploma or equivalent required.
• Must be 18+ with at least 1 year experience with frail elderly population.
• Valid NC driver’s license with clean driving record.
• Ability to operate handicap-accessible vehicles and related equipment.
• Immunizations up to date and medically cleared for direct participant contact.

Responsibilities

• Plan and coordinate transportation trips for Senior TLC participants and daily attendance.
• Determine safe and efficient pickup/delivery routes and adjust schedules with drivers.
• Communicate with participants, families, staff and agencies; attend IDT meetings as needed.
• Provide training/support to Drivers to ensure competency with various vehicles including accessible vans.
• Ensure timely delivery of participants to appointments and maintain safety compliance (wheelchair lifts, restraints).
• Document assessments and incidents in the participant's records per policy.
• Participate in care planning and help implement SMART goals for participants.
