Job summary

Kintegra Health in Charlotte, NC is seeking a Recreational Therapist to conduct initial assessments, reassessments, and care plan updates in a home and community-based model. You will communicate participant changes to the IDT and develop therapeutic activities based on need and input from the team.

Qualifications

• BS degree in Recreation Therapy required.
• Minimum 1 year experience with frail elderly population.
• Licensed Recreational Therapist in NC required.
• Immunizations up to date and cleared for communicable diseases.

Responsibilities

• Conduct all initial assessments and scheduled reassessments and communicate changes to IDT/Care Team.
• Document in the participant's medical record promptly as per policy.
• Plan, develop and implement therapeutic activities per the care plan.
• Participate in IDT/ Care Team meetings and care planning.
• Prepare monthly/daily calendar for activities in collaboration with CNAs.
• Support Senior TLC's mission to improve quality of life for seniors in the community.
