Qualifications

• High School degree or equivalent.
• Must be 18+ and have a minimum of 1 year of experience working with the frail elderly population.
• Valid NC driver’s license with a clean driving record and no major violations.
• Must be able to work effectively in a team environment and operate handicap-accessible vehicles.
• Meet a standardized set of competencies for Senior TLC and CMS before working independently and yearly thereafter.
• Immunizations up to date and medically cleared for communicable diseases.

Responsibilities

• Coordinates and plans all transportation trips for participants for Senior TLC daily attendance, specialty appointments, discharge from SNF and hospital, dialysis trips including weekends and holidays.
• Determines the safest and most efficient pick-up and delivery routes.
• Receives feedback from drivers to integrate into transportation schedules.
• Provides training and support to the team of drivers to ensure competence in driving a variety of vehicles in the fleet.
• Ensures drivers are trained to operate wheelchair lifts and tie-down equipment daily.
• Delivers participants to appointments in a timely manner.
• Attends staff meetings and communicates changes in participant condition to the Center Manager.
• Maintains a safe driving record and verifies NC license annually.
• Meets all physical requirements of the position (standing, bending, reaching, etc.).
• Participates in care planning and enters care plan information in a timely manner.
• Documents assessments, incidents, and participant information in EMR and other docs.
• Assists with special projects assigned by the Transportation Manager.
