Job summary

St. Joseph Place in Durham, NC is seeking a Part-Time Service Coordinator to support aging in place for residents of the HUD 202 Senior Housing community. You will assess needs, coordinate services, and connect residents with transportation, meals, counseling, and healthcare resources.

As a trusted advocate, you will build relationships with local agencies, develop individualized service plans, and promote a resident-centered environment that preserves dignity, independence, and quality of life.

Qualifications

• Bachelor's degree in Social Work or related field OR five (5) years of experience in social services.
• Preferred: at least one (1) year of experience working with older adults or in a geriatric setting.
• Demonstrates empathy, professionalism, and sound judgment.

Responsibilities

• Conduct assessments to identify residents' social, health, and support needs.
• Provide case management, resource referrals, and service coordination.
• Connect residents with community services such as transportation, meal programs, counseling, wellness programs, legal resources, personal assistance, and healthcare services.
• Build and maintain strong relationships with local agencies, service providers, and community partners.
• Advocate for residents and help them navigate applications, services, and available benefits.
• Develop individualized service plans and monitor ongoing support services.
• Help residents strengthen relationships with family, friends, neighbors, and community resources.
• Coordinate educational programs and training that support successful aging in place.
• Recruit and develop volunteer opportunities that enhance resident engagement and well-being.
• Partner with staff to promote a resident-centered environment that values dignity, independence, and quality of life.
