Job summary

Qualifications

Responsibilities

• Provide general case management and referral services to residents needing assistance.
• Establish linkages with community agencies to secure flexible, individualized services.
• Coordinate providers for use by staff and residents.

• Educate residents on service availability and rights; provide advocacy as appropriate.
• Develop case plans in coordination with assessment services.
• Monitor ongoing provision of services and progress with agencies.
• Establish volunteer support programs with community organizations.
• Help residents build informal support networks with family and friends.
• Provide training to residents on tenancy obligations and related topics.
