Qualifications

• Bachelor’s degree in social work or related field preferred.
• Two or more years of social services delivery for elderly or families preferred.
• Knowledge of supportive services and local community resources.
• Ability to advocate, organize, problem-solve and deliver measurable results.

Responsibilities

• Coordinate programs and activities for residents.
• Serve as liaison to community agencies and providers.
• Identify low-cost services and negotiate discounts.
• Develop a resource directory of local services.
• Sponsor educational events on health care, life skills and referrals.
• Provide quality customer service and address resident concerns.
• Assist residents in accessing community services and transportation.
• Monitor feedback from residents and families; relay to appropriate parties.
• Maintain strong relationships internally and externally.
