Job summary

St. Joseph Place in Durham, NC is seeking a Part-Time Service Coordinator to assist residents in aging in place with dignity and independence. You will conduct assessments, provide case management, and connect residents with transportation, meals, counseling, and healthcare services.

Collaborating with local agencies and staff, you will advocate for residents, develop individualized service plans, and promote a resident-centered environment that supports quality of life for older adults.

Qualifications

• Bachelor's degree in Social Work or related field, or equivalent experience.
• Minimum of five (5) years of experience in social services.
• Preferred: at least one year of experience with older adults or geriatric settings.

Responsibilities

• Conduct assessments to identify residents' social, health, and support needs.
• Provide case management, resource referrals, and service coordination.
• Connect residents with transportation, meal programs, counseling, wellness programs, legal resources, and healthcare services.
• Advocate for residents and help navigate applications and benefits.
• Develop individualized service plans and monitor ongoing support services.
