Qualifications

• Excellent customer service skills.
• Experience with seniors is a plus.
• Bachelor of Social Work or a degree in Gerontology, Psychology or Counseling preferred; relevant experience acceptable.
• Two to three years of social service delivery experience with seniors and disabled in the project area.
• Knowledge of supportive services and resources for elderly and disabled residents.
• Knowledge of referral processes.
• Ability to advocate, organize, and deliver results for elderly and disabled.
• Proficiency with MS Word, Excel and Outlook.
• Ability to work cooperatively with on-site staff, vendors and residents.

Responsibilities

• Provide general case management (including intake) and referral services to residents.
• Establish linkages with community agencies and providers.
• Set up a provider directory for staff and residents.
• Refer residents to community service providers.
• Educate residents on service availability and procedures; provide advocacy.
• Develop case plans with assessment services or PAC.
• Monitor ongoing services and update progress with providers.
• Set up volunteer support programs with community organizations.
• Help residents build informal support networks.
